Our Mission

By listening and responding to the needs of our community, we aim to provide high-quality, inclusive healthcare and support, delivered by a committed, effective, and caring team in a safe environment, with time for all.

Our Vision

To work in conjunction with the NHS vision of enabling people to live healthier and more independent lives through high-quality integrated care and preventative medicine.

Our Values

  • Kindness, Empathy, and Respect: We treat everyone with dignity and understanding.
    • Integrity and Excellence: We uphold honesty, ethical behaviour, and strive for continuous improvement.
    • Teamwork and Innovation: We work collaboratively, embrace new ideas, and take responsibility for delivering transparent, high-quality care.

Our Locations & Facilities

Appletree Medical Practice operates from a purpose-built building in Duffield, with a branch surgery in Little Eaton open each morning. Both sites are accessible and compliant with Health & Safety and disability regulations, providing a safe, clean, and welcoming environment for patients and staff.

Our Service Area

We provide NHS primary care services to Duffield, Little Eaton, and surrounding villages, serving patients of all ages and backgrounds. We are part of the Belper Primary Care Network (PCN), working collaboratively with neighbouring practices to improve access, coordinate care, and develop services that meet local population health needs.

Understanding Our Population

We use local demographic data, including information from the Derbyshire Joint Strategic Needs Assessment (JSNA) and SHAPE Atlas, to understand the health needs of our community. This allows us to identify health inequalities and proactively support vulnerable groups, such as people living with long-term conditions, frailty, or mental health needs.

Regulated Activities

We are registered with the Care Quality Commission (CQC) to provide the following regulated activities:
• Diagnostic & Screening Procedures
• Family Planning
• Maternity & Midwifery Services
• Surgical Procedures
• Treatment of Disease, Disorder or Injury

How We Deliver Care

  • We involve patients in decisions about their care and actively encourage feedback through our Patient Participation Group, Friends and Family Test, Google Reviews, and Complaints and Suggestions processes.
    • We are committed to equality, diversity, and inclusion, ensuring non-discriminatory care for all.
    • We maintain a safe, clean, and suitably equipped environment.
    • Our staff receive ongoing training, supervision, and professional development.
    • We work closely with other health and social care providers, including our PCN multidisciplinary teams.
    • We have robust systems for safeguarding, infection prevention and control, medicines management, and clinical governance.
    • We welcome and act on feedback and learning from incidents to continuously improve services.
    • We ensure confidentiality and data protection in line with legal and NHS requirements.

Access and Digital Care

We offer a range of access routes, including online consultations, telephone, and face-to-face appointments, ensuring equitable and timely access for all. Our website provides self-help resources, care navigation tools, and links to local support services, enabling patients to make informed choices about their care.

Quality Improvement

We are committed to continuous quality improvement through regular clinical audits, significant event reviews, patient safety reviews, and learning from complaints. Findings are shared across the practice and the Belper PCN to enhance patient safety, clinical outcomes, and service efficiency.

Our People

We value and invest in our staff, recognising that a supported, skilled, and motivated team is essential to delivering high-quality patient care. We promote a positive, inclusive culture that encourages open communication, professional growth, wellbeing, and teamwork.

Governance & Compliance

Appletree Medical Practice complies with all relevant NHS, CQC, and legislative requirements. We maintain clear systems for clinical governance, data protection, safeguarding, health and safety, and risk management. Our Statement of Purpose is reviewed regularly and updated when changes occur, with the CQC notified as required.

Our Commitment

Our aim is to ensure that every patient receives compassionate, safe, and effective care, supporting their health, wellbeing, and independence throughout life.
